ClinGen expert panel — Monogenic Diabetes VCEP

Likely Pathogenic for monogenic diabetes. Applied ACMG criteria (verbatim):

PP4_Moderate PM2_Supporting PS2_Supporting PS4_Moderate PM1 PP2 PP3

Curation rationale (ClinGen, verbatim)

The c.451T>C variant in the glucokinase gene, GCK, causes an amino acid change of serine to proline at codon 151 (p.(Ser151Pro)) of NM_000162.5. GCK is defined by the ClinGen MDEP as a gene that has a low rate of benign missense variation and has pathogenic missense variants as a common mechanism of disease (PP2). This variant is predicted to be deleterious by computational evidence, with a REVEL score of 0.991, which is greater than the MDEP VCEP threshold of 0.70 (PP3). This variant has an incomputable gnomAD v4.1.0 Grpmax filtering allele frequency due to no copies in the European non-Finnish subpopulation and one copy in any other subpopulation, thereby meeting the ClinGen MDEP threshold criteria for PM2_Supporting (Grpmax FAF <= 0.000003) (PM2_Supporting). This variant resides in an amino acid that directly binds glucose, which is defined as critical for the protein’s function by the ClinGen MDEP (PM1). This variant was identified in four unrelated individuals with hyperglycemia (PS4_Moderate; PMID: 19564454, 36257325, internal lab contributors). This variant was identified in an individual with a clinical history highly specific for GCK-hyperglycemia (fasting glucose 5.5-8 mmol/L and HbA1c 5.6 - 7.6% and negative antibody) (PP4_Moderate; internal lab contributors). This variant was identified as a de novo occurrence with unconfirmed parental relationships in an individual with hyperglycemia, but with insufficient clinical information to determine if the phenotype is highly specific for GCK-hyperglycemia (PS2_Supporting; internal lab contributors). In summary, c.451T>C meets the criteria to be classified as likely pathogenic for monogenic diabetes. ACMG/AMP criteria applied, as specified by the ClinGen MDEP (specification version 3.1.0, approved 10/10/2025): PS4_moderate, PP4_Moderate, PM1, PS2_Supporting, PM2_supporting, PP2, PP3.

AlphaMissense carries the ACMG weight for missense; REVEL and SaProt are independent agreement signals.

Rarity supports pathogenicity (ACMG PM2); frequency above a disease-specific BA1/BS1 threshold argues against it. A variant frequent in one ancestry group can be near-absent in others, so a single global figure can mislead.
